Insurance
Your insurance will be verified by our staff prior to your appointment or procedure to determine eligibility, deductibles and patient co-payments and co-insurance responsibility. All payments are due at the time of your appointment.
You should also bring your insurance card with you to each appointment. If your health plan should change while you are a patient, please let us know so we can update our records.
If your insurance company requires a referral, please call your primary care physician prior to your appointment to arrange for it.
You will be responsible for paying any balance that is not covered by your policy. Charges for routine, elective procedures that are not covered by your insurance policy will be due prior to the procedure. Payment plans are available for urgent/emergency procedures that are not covered by insurance. Self-pay patients will be given an estimate of their charges prior to their appointment or procedure.
Accepted payment methods include cash, check, Mastercard, Visa and American Express. If it becomes necessary to give your account to our collection agency, we may reschedule any routine/non-urgent visits until the balance is paid.
For Obstetrical Patients: your insurance coverage will be verified after your initial visit. You will be notified of your financial responsibility and a payment plan will be set up. Full payment is required prior to delivery. See separate Obstetrical Financial Policy, which outlines what is included in the global fee.
Our office accepts most insurance policies. If your plan is not listed, please call our Billing Department for more information as we are here to assist you as needed.
