Appointment Info

Appointments can be scheduled through our main telephone number or via our Communicator Patient Portal (located on the left side of this screen).  All patients are seen by appointment only.

We make every effort to accommodate patient requests for certain providers; however, some of our providers at not in the office every day. If your appointment is of an urgent nature, you may be asked to see another provider.  Patients with an urgent problem are seen that day, referred to Lowell General Hospital’s Emergency Department or the Labor and Delivery floor.

In order to provide you with optimal care, it is important that all medical and insurance information is completed before your scheduled appointment.  If your information is not completed prior to your appointment, it may be necessary to reschedule your appointment.

Appointment Checklist:

  1. Bring your insurance card with you to each appointment.
  2. Update your demographics online using our Communicator Patient Portal (located on the left side of this screen)
  3. Update your medical history on the form sent to you with your reminder letter.
  4. Please do not wear perfumes or scented lotions to your appointment.

In the event of inclement weather, please call us to determine if the office is open.

New Patients

To better serve you, please complete the following paperwork prior to your new patient appointment.

** The Consent to Release Medical Records form must be completed and forwarded to your previous doctor’s office.  This form provides your permission to obtain your medical records.  Copying and sending your previous medical history can take a few weeks to process.  Please send out the Consent to Release Medical Records as soon as possible.

If you are unable to complete the above information prior to your appointment, please arrive twenty (20) minutes early. If your paperwork is not complete and you do not arrive early to fill out the forms, your appointment may need to be rescheduled.

If you have problems with our online Patient Portals or have any questions regarding the registration process, please do not hesitate to call us during normal office hours.

Current Patients

Before your scheduled visit with us, we request that you:

  1. Update your demographics online using our Communicator Patient Portal (located on the left side of this screen)
  2. Update your medical history by completing the form sent to you
  3. Update your insurance provider information online using our Communicator Patient Portal (located on the left side of this screen)

Running Late/Cancellations/No Shows

We value our patients’ time. If you are running late for your appointment, please call ahead. If you are more than 15 minutes late, our policy is to reschedule your appointment. If you need to cancel your appointment, please call us as soon as possible.

Please note that you will be charged a $25.00 fee if you do not cancel your appointment within 24 hours.
